About the PS 1508 Statement by Shipper of Firearms

As of 18 February 2013, the USPS website indicates:

"PS Form 1508 is used by firearms manufacturers, dealers, or importers for shipping merchandise, parts, replacement parts of firearms. Once filled out, it is retained by the United States Postal Service for one year."

The PS 1508 Statement by Shipper of Firearms form shows:

The date and ship-from city, state, and ZIP Code (top of form).
The address details of the packages.
Fields that you and the USPS representative must sign, date, or stamp when you drop off your mailpieces for shipment.
